We pride ourselves in offering the most advanced training in Reproductive Endocrinology and Infertility. We look forward to working with you and continuing to advance patient care within the Atlanta community.

Simple ways you can refer your patient for a virtual new patient appointment or an outside testing appointment (Semen Analysis or HSG):

1. Email Our Scheduling Team

2. Fax Referral Form

Print and fill out the below PDF and fax to 678.303.0482 or email scheduling@acrm.com. This form should be used for outside semen analysis and HSG appointments. Please note: ACRM does not offer Semen Freezing. We cannot schedule your patient for an outside testing appointment without a referral that includes a physician's signature and a diagnostic code.

In an effort to ease patient pain, anxiety, and discomfort, ACRM offers Pro-Nox for HSG procedures. Your patient can request this during the scheduling process.
