We pride ourselves in offering the most advanced training in Reproductive Endocrinology and Infertility. We look forward to working with you and continuing to advance patient care within the Atlanta community.

Simple ways you can refer your patient for a virtual new patient appointment or an outside testing appointment:

1. Email Our Scheduling Team

Email scheduling@acrm.com.

2. Fax Referral Form

Print and fill out the below PDF and fax to 678.303.0482 or email scheduling@acrm.com. This form should be used for outside semen analysis and HSG appointments. Please note: ACRM does not offer Semen Freezing. We cannot schedule your patient for an outside testing appointment without a referral that includes a physician's signature and a diagnostic code.

3. Contact Phreesia to Establish a Connect Account

This allows your practice to connect via a dashboard directly with ACRM. There you can submit referrals, attach any important documents, and rest easy knowing your patient will be proactively contacted by one of our New Patient Coordinators.

4. Use A Live Interactive Form

Please fill out the referral form below. By filling this form out, you will be submitting an online appointment request that instantly populates to the Phreesia Connect Dashboard.
